- Checklist for registration of a credit reporting agency
- Applicant's covering letter on company letterhead, signed by the chief executive or any director of the applicant
- Application Form for Registration of Credit Reporting Agency (Form PPK 1)
- Statutory declaration by the chief executive and every director of the applicant fulfilling the criteria outlined in the Third Schedule to the Act
- List and description of other credit reporting agencies that have a relationship in the shareholding structure or interest of the applicant
- Categories and description of the applicant’s current and prospective customers
- Categories and description of parties having a relationship with the applicant concerning credit reporting activities, collection, and dissemination of credit information, including but not limited to resellers
DOCUMENT SUBMISSION
The fully completed Application Form for Registration of Credit Reporting Agency (Form PPK 1), along with the prescribed fee, required documents, and the checklist, must be submitted to the following address:
